- EU citizens, citizens of Norway, Iceland, Liechtenstein, Switzerland, or family members of EU citizens residing in Poland with a permanent residence permit or long-term EU resident status

- Personal details of the inviting person (name, date of birth, citizenship, address)
- Personal details of the invited foreigner (name, date of birth, citizenship)
- Purpose and planned duration of the foreigner's stay in Poland
- Proof that the inviting person has sufficient financial resources to support the foreigner in Poland
- Document confirming the inviting person's legal right to the accommodation where the foreigner will stay (e.g., property deed, rental agreement)
