This form is used by foreigners who want to apply for a European Union (EU) long-term residence permit in Poland.
The residence permit of a long-term resident of the EU entitles a foreigner to:
Live in Poland for an unlimited time
Work without any restrictions
Study
Engage in entrepreneurial activities
Move freely in EU countries
To apply for a long-term EU resident's residence permit, a foreigner must:
It is legal to stay in Poland for 5 years
Have a constant source of income
Have health insurance
Have a command of the Polish language at a level not lower than B1
Not to pose a threat to the public order and security of Poland
The following must be added to the application:
Valid passport
A document confirming a legal stay in Poland for 5 years
A document confirming the source of income
A document confirming the availability of medical insurance
Certificate confirming knowledge of the Polish language
Other documents provided by law
The application is submitted to the voivode at the place of residence of the foreigner.
The application review process can take up to 3 months.
